The bolt typically locks forward when a piece of shell from a chopped ball gets stuck between the bolt and the chamber. This jams the bolt in the forward position, and causes the ram to continue to press against the cup seal shaft, also causing air to escape from the barrel. The quick fix for this is to pull back on the bolt knob, squeegee the barrel, and clean things up after the game.

However, your post doesn't mention chopped paint. If this isn't the problem, I would check the functioning of the ram (is it lubed, does it slide back and forth smoothly, is the spring working smoothly?) and clean and adjust the LP regulator.
